Kieferprobleme

Kieferprobleme is a German term used to describe disorders involving the jaw, the temporomandibular joints (Kiefergelenke) and the masticatory muscles. They are commonly encountered in dentistry, oral and maxillofacial medicine, and pain management, and can affect chewing, speaking and daily function.

The most frequent forms are temporomandibular joint disorders (TMD or TMJ disorders), myofascial or muscle-related pain, and problems arising from malocclusion or misalignment of the teeth and jaws. Other contributors can include jaw injuries, arthritis in the jaw joints, disc displacement within the joint, bruxism (teeth grinding), or after dental procedures and orthodontic treatment. Kieferprobleme can present as intermittent or persistent pain, restricted jaw movement, clicking or popping sounds, headaches, ear fullness or sensitivity in the face.

Causes and risk factors are varied and often multifactorial. They may involve mechanical overload from grinding or clenching, stress, occlusal disharmony, previous trauma, inflammatory or degenerative joint disease, and certain structural or developmental jaw conditions. Poor posture and neck or facial muscle tension can also contribute to symptom development.

Diagnosis relies on a clinical examination by a health professional, often supported by imaging such as panoramic radiographs, CT or MRI to assess joint structure, disc position and surrounding tissues. Differential diagnosis may exclude dental infections, sinus issues or neurological conditions.

Treatment is typically multimodal and tailored to the underlying cause. Conservative approaches include patient education, soft food diet, heat or cold therapy, nonsteroidal anti-inflammatory drugs, analgesics, bite splints or night guards, and physical therapy with jaw exercises. Addressing malocclusion, reducing bruxism, stress management and good oral hygiene are common components. In refractory or severe cases, dental procedures, injections into the joint, arthroscopic surgery or, rarely, joint reconstruction may be considered.
