Fehlbiss

Fehlbiss, a German term, refers to malocclusion in dentistry: an abnormal relationship between the upper and lower teeth when the jaws close. It describes misalignment of teeth, irregular bite patterns, and sometimes jaw position that prevents proper contact between dental arches. Malocclusion can affect chewing efficiency, speech, tooth wear, and facial aesthetics.

Causes of Fehlbiss include genetic factors, developmental changes, early loss of primary teeth, and habits such as thumb sucking or tongue thrusting. Malocclusions are typically categorized as dental (tooth-position problems with otherwise compatible jaws) or skeletal (discrepancies in jaw size or position). Common bite patterns include overbite (deep bite), underbite (prognathism), crossbite, open bite, crowding, and spacing. In dentistry, Angle’s classification is often used to describe dental occlusion: Class I (neutral jaw relationship with some misalignment), Class II (retrognathic mandible, with Division 1 or Division 2 subtypes), and Class III (prognathic mandible).

Symptoms and diagnosis: People with Fehlbiss may experience difficulty chewing, uneven tooth wear, jaw or temple headaches, speech issues, or cosmetic concerns. Diagnosis combines clinical examination with bite analysis, X-rays (such as panoramic and cephalometric images), photographs, bite registrations, and sometimes dental impressions or 3D scans to assess tooth alignment and jaw relationships.

Treatment and prognosis: Management depends on cause and severity. Mild cases may be treated with orthodontic appliances such as braces or clear aligners, while interceptive treatment in children can guide jaw development. Severe skeletal discrepancies may require orthognathic (jaw) surgery in combination with orthodontics. In adults, treatment often involves orthodontics with restorative options or surgery. A multidisciplinary team—often including orthodontists, oral surgeons, and prosthodontists—plans care. Early detection generally improves outcomes, and not all malocclusions require treatment, but significant functional or aesthetic concerns are more likely to be addressed.
