Seletamatuks, also known as "selective mutism," is a childhood-onset anxiety disorder characterized by a consistent failure to speak in specific social situations despite the ability to speak in other contexts. This condition typically emerges before the age of five and often persists into adolescence or adulthood if left untreated. Individuals with selective mutism may communicate effectively in familiar settings, such as at home with close family members, but remain largely silent in unfamiliar or socially demanding environments, such as school or public places.

The exact cause of selective mutism is not fully understood, but it is believed to result from a combination of genetic, neurological, and environmental factors. Some researchers suggest that children with selective mutism may have heightened sensitivity to social cues or an increased fear of negative evaluation. Trauma, such as bullying or a significant life change, can also contribute to the development or exacerbation of the condition. Additionally, co-occurring disorders like social anxiety disorder, separation anxiety, or other anxiety-related conditions are common in individuals with selective mutism.

Diagnosis of selective mutism involves a comprehensive evaluation by a mental health professional, such as a psychologist or psychiatrist. The assessment typically includes interviews with the child, caregivers, and teachers, as well as observations of the child’s behavior in different settings. Differential diagnosis is crucial to rule out other conditions that may present with similar symptoms, such as autism spectrum disorder, hearing impairments, or language disorders.

Treatment for selective mutism often involves a combination of behavioral therapies, such as exposure therapy and social skills training, to gradually reduce anxiety in social situations. Cognitive-behavioral therapy (CBT) is frequently used to help individuals identify and challenge negative thought patterns. In some cases, medication may be prescribed to manage associated anxiety symptoms, though it is not typically the primary treatment. Early intervention and a multidisciplinary approach, involving parents, educators, and healthcare providers, are key to improving outcomes for individuals with selective mutism.
