Onychocryptose is a dermatological condition characterized by the local impeding of a toenail, often resulting in inflammation, pain, and potential infection. The term, derived from the Greek roots onycho (toenail) and cryptos (hidden), describes a scenario in which the toe becomes enveloped by the surrounding skin. While onychocryptosis refers to an ingrown toenail, onychocryptose specifically highlights the impingement and ensuing inflammatory response that may develop secondary to altered nail growth or external mechanical factors.

The condition most commonly affects the great toe, particularly in women, but it may also involve other digits. Risk factors include poorly fitting footwear that exerts pressure on the toe, fungal infections that alter nail thickness, hereditary nail anomalies, extensive nail trauma, or repeated intrusive nail clipping. Symptoms typically involve tenderness, erythema, local swelling, occasional purulent drainage, and impaired mobility of the affected foot.

Diagnosis comes from a careful physical examination, sometimes supplemented by imaging to rule out underlying bony abnormalities. In chronic cases, a skin culture may be taken to detect pathogenic organisms. Treatment priorities focus on alleviating pain, eliminating infection, and restoring normal nail growth. Conservative measures involve warm soaks, proper nail trimming with a straight edge, use of soft-soled shoes, and topical or oral antibiotics if secondary infection is present. Advanced cases may necessitate simple surgical intervention, such as partial nail avulsion, to relieve impingement. Post‑operative care includes maintaining hygiene, wearing appropriate footwear, and monitoring for recurrence.
