Logopenic-phonological, often described as the logopenic variant of primary progressive aphasia (lvPPA), is a clinically defined pattern of language impairment arising from a progressive neurodegenerative process. The term reflects two core features: logopenia, or slowed, hesitant speech with word-finding pauses, and phonological processing difficulties that produce phonological errors in speech and repetition.

Clinical features. Early in the course, speech is fluent but marked by pauses and misnamed or missing words. Repetition of phrases and sentences is disproportionately impaired, reflecting short-term phonological memory deficits. Word retrieval is impaired, while grammar and single-word comprehension are relatively preserved initially. Reading and writing may become affected as the disease advances, primarily due to phonological and memory impairments. Nonlanguage cognitive domains are often comparatively spared in the early stages but may decline later.

Diagnosis and biomarkers. lvPPA is diagnosed through clinical evaluation supported by neuropsychological testing showing prominent deficit in repetition and phonological processing with relatively preserved single-word comprehension and grammar. Neuroimaging typically reveals left temporoparietal atrophy and hypometabolism, especially in the posterior perisylvian region. Most cases are associated with Alzheimer’s disease pathology, as indicated by amyloid biomarkers, though atypical underlying pathologies can occur. It is important to distinguish logopenic-phonological variants from semantic and nonfluent/agrammatic variants of primary progressive aphasia, which present with different language profiles and imaging patterns.

Management and prognosis. There is no disease-modifying therapy for lvPPA; management focuses on supportive care and speech-language therapy targeting phonological processing, word retrieval strategies, and communication strategies for daily life. Regular assessment and caregiver education are integral to care.
