Diseasemodifying, commonly written as disease-modifying, is a term used in medicine to describe interventions that aim to alter the natural history of a disease rather than simply relieving its symptoms. A disease-modifying approach seeks to slow or halt progression, reduce irreversible damage, and improve long-term function by targeting the underlying pathophysiology that drives the condition.

The scope of disease modification includes pharmacologic therapies, biologic agents, and, in some areas, lifestyle, surgical, or device-based strategies that influence disease mechanisms. In rheumatology, disease-modifying antirheumatic drugs slow joint damage in rheumatoid arthritis. In neurology, disease-modifying therapies intend to reduce relapses and disability in multiple sclerosis. In other chronic diseases, research continues into interventions that change biomarkers, imaging findings, or clinical trajectories in an effort to alter long-term outcomes.

Assessment of disease modification relies on long-term evidence showing a change in disease trajectory rather than short-term symptom relief. Trials often use endpoints such as radiographic progression, functional status, and time to disability, with biomarkers sometimes serving as surrogate measures. Regulatory and clinical interpretations of disease modification vary by disease area, and not all conditions have proven disease-modifying options.

Limitations include variability among patients, the need for extended follow-up, and the balancing of benefits against risks. The magnitude of modification may differ with disease stage, patient characteristics, and comorbidities. Nonetheless, disease modification remains a central concept in evaluating therapies intended to change the course of illness, beyond symptom control.
