TumorStaging

TumorStaging is the systematic assessment of the extent of cancer within the body. It describes the size and location of the primary tumor, whether regional lymph nodes are involved, and whether distant sites have been affected. The most widely used framework is the TNM system, developed and regularly updated by organizations such as the AJCC and the UICC. Staging helps predict prognosis and guide treatment decisions.

In the TNM system, T refers to the size and extent of the primary tumor, N to involvement of regional lymph nodes, and M to distant metastasis. Combinations of T, N, and M are used to assign an overall stage, typically from Stage 0 (carcinoma in situ) to Stage IV (advanced metastatic disease). Stage grouping reflects overall tumor burden and spread rather than a single element.

Staging can be clinical (based on physical exam, imaging, and noninvasive tests) or pathological (based on examination of tissue removed during biopsy or surgery). Pathological staging often yields more precise information and may upstage a cancer after surgical findings. Some cancers also include subcategories (such as T1, N1, M1) to convey exact details.

Assessment usually employs imaging methods such as computed tomography, magnetic resonance imaging, positron emission tomography, and ultrasound, along with biopsies and sometimes sentinel lymph node biopsy. Restaging after therapy helps evaluate response and detect progression.

Staging systems are cancer-specific and may be revised as new data emerge. In addition to solid tumors, hematologic malignancies use different criteria. Staging does not capture all prognostic factors; tumor grade, molecular markers, patient performance status, and treatment considerations also influence outcomes and choices. Limitations include sampling error and the presence of undetected micrometastases. Ongoing research incorporates molecular and genetic information to refine staging. Regular updates ensure that staging reflects current understanding and improves consistency across institutions.
