Outcomes can include charges or exoneration, disciplinary actions, policy changes, settlements, or corrective measures. Investigations face challenges such as incomplete or biased evidence, jurisdictional limits, resource constraints, and time pressures, but rely on methodological rigor, corroboration, and transparent documentation.
