Sebum-associated is a descriptor used in dermatology to refer to features, conditions, or processes that are linked to or influenced by sebum—the lipid-rich substance produced by sebaceous glands. It is not a formal disease name but rather a functional category for observations tied to sebaceous activity, particularly in regions with high gland density such as the face, scalp, and upper chest.

Sebum is composed of triglycerides, wax esters, free fatty acids, squalene, and cholesterol esters. Its production is influenced by genetic factors and hormonal signals, especially androgens. Sebum production tends to be highest during adolescence and may fluctuate with age, hormonal status, and environmental factors. The amount and composition of sebum can shape the skin’s surface characteristics and its microbial milieu.

Clinical relevance emerges in several conditions described as sebum-associated. Acne vulgaris is commonly viewed as sebum-associated due to excess sebum production, altered keratinization, and bacterial colonization by Cutibacterium acnes, which can drive inflammation. Seborrheic dermatitis and oily skin are also linked to heightened sebum activity; the Malassezia yeast that colonizes sebaceous-rich areas often participates in the condition’s pathophysiology. The skin microbiome is influenced by sebum, as lipid-rich environments favor lipophilic microbes and can modulate host immune responses.

Assessment of sebum-related features can involve clinical examination and, in research contexts, sebumetry to estimate sebum excretion rates. Management strategies aiming to modulate sebum production include topical retinoids, azelaic acid, barrier-friendly cleansers, hormonal therapies in appropriate individuals, and, in more severe cases, systemic agents that reduce sebaceous gland activity.
