Phobias are a category of anxiety disorders characterized by an excessive and persistent fear of a specific object or situation. The fear is out of proportion to actual danger and leads to avoidance, distress, or impairment in daily functioning. Phobias commonly begin in childhood or adolescence and affect both sexes, with higher reported rates in women.

The main types are specific phobias (animals, natural environment, blood-injection-injury, or situations such as flying or doors), social anxiety disorder (social phobia), and agoraphobia. A subtype, blood-injection-injury phobia, can cause fainting due to a distinct physiological reaction.

Diagnosis is based on criteria such as a marked fear that is excessive for the context, lasting six months or more, and leading to avoidance or severe distress. The fear is not better explained by another mental disorder and is not due to a medical condition or substance.

Most phobias arise from a combination of genetic, temperamental, and environmental factors. Learning histories, traumatic events, and evolutionary preparedness contribute to vulnerability to certain stimuli. Family history and prior exposure can influence development and persistence.

Treatment is typically cognitive-behavioral, emphasizing exposure to the feared stimulus in a gradual, structured way (systematic desensitization or graduated exposure). CBT, sometimes with virtual reality tools, is effective. Medications may be used in some cases to target anxiety symptoms but are not first-line for isolated phobias; SSRIs or SNRIs are the most common pharmacologic options.

Epidemiologically, phobias are common disorders with substantial variability by population. With appropriate treatment, many patients improve significantly; however, untreated phobias can impair education, work, and social functioning and may co-occur with other anxiety or mood disorders.
