Isjias is a term that in some languages or regional usage refers to the condition commonly known in English as sciatica or ischialgia. It is not a distinct medical diagnosis in standard English-language references, but rather a name used for symptoms arising from irritation or compression of the sciatic nerve. Sciatica describes the collection of symptoms that follow the pathway of the sciatic nerve, which extends from the lower back through the buttock and down the leg.

Symptoms typically include unilateral pain in the lower back, buttock, and leg, often described as sharp, burning, or electric in quality. Some people experience numbness, tingling, or muscle weakness in the affected leg. Pain may worsen with movement, coughing, or prolonged sitting, and may improve with rest. Presentation and severity vary widely.

Causes of sciatica, and thus of isjias in usage, include lumbar disc herniation, spinal stenosis, degenerative disc disease, spondylolisthesis, piriformis syndrome, pregnancy, infections, or tumors. Risk factors encompass age-related wear, obesity, sedentary lifestyle, smoking, and certain occupations that involve heavy lifting or twisting.

Diagnosis is typically clinical, based on history and neurological examination, supported by imaging such as MRI or CT when indicated. Nerve conduction studies may help in complex cases. Treatment focuses on pain relief and function: nonsteroidal anti-inflammatory drugs, acetaminophen, physical therapy, activity modification, and hot/cold therapy. Epidural steroid injections or nerve blocks may be considered for persistent symptoms. Surgical options, like discectomy or laminectomy, are reserved for severe or progressive neurological deficits or failure of conservative care. Most cases improve within weeks to months with appropriate management.
