Organizations use problemsolutions to address inefficiencies, quality issues, safety concerns, and strategic challenges. The process is typically iterative, with feedback loops used to refine understanding and prevent recurrence.

Common challenges include cognitive biases, incomplete information, resistance to change, organizational politics, and difficulties in measuring outcomes. Ethical considerations and unintended consequences should also be examined when selecting and implementing solutions.
