Definitions and scope: The term encompasses both low levels of purposeful exercise and long periods of sedentary time. It is distinct from undernutrition or disease-related immobilization, though often co-occurs with obesity.

Physiological mechanisms: Inactivity reduces muscular strength and mass, deconditioning of cardiovascular and metabolic systems, decreased bone remodeling, impaired insulin sensitivity, dyslipidemia, and low-grade inflammation. These changes can accumulate and elevate risk for cardiovascular disease, type 2 diabetes, certain cancers, osteoporosis, and functional decline.

Impact and populations: Inactivity-related health risks affect all ages but increase with age and may be pronounced among desk workers, caregivers, and those with mobility limitations. Even individuals with normal body weight can exhibit inactivity-related cardiometabolic risk if sedentary.
