Pointshyperirritable is a term used to describe discrete, localized points within soft tissue that exhibit increased sensitivity of nociceptors and produce pain upon palpation, sometimes with referred pain patterns. The concept combines the idea of a pinpoint site with hyperirritability of neural and inflammatory mediators, and it is discussed mainly in the context of trigger-point phenomena.

The term is not widely adopted in major medical classifications and is chiefly encountered in clinical descriptions of myofascial pain features. It is often used in relation to myofascial trigger points and may be applied to describe active points that provoke pain or to latent points that are tender without spontaneous pain.

Clinical features typically include focal tender points found in muscles or fascia, commonly located in areas such as the neck, shoulder, jaw, or hip regions. Palpation of these points reproduces the patient’s pain, and accompanying symptoms may include stiffness, reduced range of motion, and, less commonly, mild autonomic changes in the surrounding area.

Pathophysiology theories emphasize local factors such as minor muscle trauma, sensitization of peripheral nociceptors, and the release of inflammatory mediators, which can lead to taut bands and altered muscle spindle activity. Central sensitization may also contribute to amplified pain and the spread of referred pain patterns.

Diagnosis is clinical, relying on patient history and physical examination. There are no universally accepted objective tests for pointshyperirritable points, and imaging is rarely necessary. Differential diagnoses include myofascial trigger points, fibromyalgia, radiculopathy, and neuropathic pain, depending on distribution and accompanying features.

Management tends to be multimodal and conservative, including physical therapy with stretching and manual techniques, dry needling or injections by trained clinicians, nonsteroidal anti-inflammatory drugs as appropriate, and patient education on posture and activity modification. Many cases improve with consistent treatment, though persistent or widespread involvement can be more challenging.
