Osservazionale is an Italian adjective meaning related to observation. In scientific contexts, it describes research that relies on systematic observation of phenomena in real-world conditions, without intentional manipulation of variables by the researcher.

In medicine and epidemiology, osservazionale studies examine associations between exposures and outcomes as they occur naturally. Common designs include cohort studies (prospective or retrospective), case-control studies, and cross-sectional studies. Data may come from medical records, administrative databases, questionnaires, or surveillance systems. In astronomy, the term can also describe studies based on telescope measurements and observational data rather than theoretical modeling.

Compared with experimental studies, osservazionale research cannot, by design, establish causality with the same certainty due to potential confounding and biases. It often offers greater external validity and feasibility, particularly when randomization is unethical or impractical. Limitations include confounding factors, selection bias, information bias, and difficulties in establishing temporality (especially in cross-sectional designs).

Analytical methods include multivariable regression, matching and stratification, and, in longitudinal designs, time-to-event analyses. Ethical considerations include privacy and informed consent when identifiable data are used. Reporting guidelines such as STROBE are commonly applied to improve transparency.
