Distrofian is a term encountered in limited medical literature to describe a dystrophic muscle and connective tissue syndrome. It is not widely recognized as a distinct disorder in major classifications, and its use varies between sources. The name is derivable from Greek roots meaning bad nourishment and a condition pertaining to dystrophy.

Classification and terminology are unsettled; some authors describe distrofian as a spectrum with subtypes based on predominant muscle involvement or connective tissue features. Because there is no consensus, the term often overlaps clinically and histologically with established muscular dystrophies and myopathies.

Clinical features typically begin in adolescence or early adulthood. Proximal symmetric muscle weakness, difficulty rising from a chair, and a waddling gait are common. Joint contractures, especially of the ankles and knees, may develop. Creatine kinase levels can be variably elevated. Electromyography usually shows a myopathic pattern, and muscle biopsy may reveal dystrophic changes such as fiber splitting and increased connective tissue.

Pathophysiology remains unproven; proposed mechanisms include defects in dystrophic pathways affecting muscle fibers and extracellular matrix remodeling, with possible inflammatory involvement. Genetic heterogeneity is suspected, but no definitive causative variant has been established for distrofian.

Diagnosis relies on clinical evaluation, laboratory testing, and electrophysiology, with muscle biopsy supporting a dystrophic process. Genetic testing is used to rule out other muscular dystrophies; a specific distrofian genotype is not defined in standard panels.

Management is supportive and multidisciplinary. Interventions emphasize physical therapy, orthoses, contracture management, and monitoring of cardiac and respiratory function, along with pain control and genetic counseling. Prognosis is variable, ranging from slow progression to more significant disability over time. The term remains rare and not uniformly adopted in formal medical taxonomy.
