Coccydynia is a medical term for pain in the coccyx, also known as the tailbone. This pain is typically localized to the area of the coccyx, and it can range from a dull ache to a sharp, shooting sensation. The pain is often exacerbated by sitting for extended periods, especially on hard surfaces, and can be relieved by standing or lying down. Certain activities like bowel movements or intercourse can also aggravate the pain.

The causes of coccydynia are varied. Direct trauma to the coccyx, such as from a fall or a direct blow, is a common cause. Repetitive strain, such as from prolonged sitting on an unsupportive surface or cycling, can also lead to coccydynia. In some cases, the cause may be related to childbirth, where the coccyx can be injured during delivery. Less common causes include infections, tumors, or arthritis affecting the coccygeal joint. Often, however, the exact cause of coccydynia remains unknown, a condition referred to as idiopathic coccydynia.

Diagnosis of coccydynia usually involves a physical examination and a review of the patient's medical history. Imaging tests such as X-rays or MRI may be used to rule out other conditions and to visualize the coccyx. Treatment for coccydynia typically starts with conservative measures. These include pain relievers like NSAIDs, avoidance of prolonged sitting, and the use of specialized cushions designed to relieve pressure on the tailbone. Physical therapy, including exercises to strengthen pelvic floor muscles, can also be beneficial. In persistent cases that do not respond to conservative treatments, injections of local anesthetics or corticosteroids may be considered, and in rare instances, surgical removal of the coccyx (coccygectomy) might be performed.
