Allostatic refers to allostasis, a concept in physiology and psychobiology that describes how the body achieves stability through change. In this view, regulatory systems such as the autonomic nervous system, the hypothalamic-pituitary-adrenal axis, and the immune system continually adjust set points and responses in anticipation of challenges, rather than simply returning to fixed baselines. Allostasis emphasizes the brain’s role in predicting needs and mobilizing resources to meet them, sometimes via rapid hormonal, cardiovascular, or metabolic changes.

Allostatic load refers to the cumulative physiological cost of chronic exposure to fluctuating or heightened neural or neuroendocrine responses. Repeated or prolonged activation of stress responses can produce wear and tear across multiple organ systems, increasing risk for cardiovascular disease, metabolic disorders, obesity, depression, and cognitive decline. The concept integrates diverse biomarkers across cardiovascular, metabolic, immune, and neuroendocrine systems; common research approaches construct an allostatic load index from such measures. The term was introduced by Sterling and Eyer (1988) with allostasis, and later refined by McEwen and Stellar (1993) to describe load or burden from chronic stress.

Applications and debates: Allostasis broadens the understanding of health beyond homeostatic set-points, but measurement validity and clinical utility remain topics of debate. It has influenced research in aging, psychiatry, public health, and preventive medicine, highlighting how chronic stress exposure can contribute to disease via dysregulated adaptation. Interventions focusing on stress reduction, sleep, physical activity, and social support are discussed as ways to mitigate allostatic load.
