Postandcore

Postandcore is a dental restoration technique used primarily to rebuild and reinforce severely damaged or structurally compromised teeth, often following the removal of a root canal. The procedure involves placing a prefabricated or custom-made post within the root canal, which serves as a connector between the remaining tooth structure and the final restorative crown. Over the post, a core material is added to build up the tooth’s structure to accommodate a crown or other prosthetic.

The main purpose of postandcore is to provide additional support and stability for a crown, especially when the natural tooth structure is insufficient to retain a restoration. It effectively distributes occlusal forces along the length of the root, reducing the risk of fracture. The process typically involves removing any remaining decay or damaged tissue, fitting and cementing the post into the root canal, and then building up the core with composite resin or other suitable materials.

There are different types of posts used in postandcore procedures, including metal posts (e.g., stainless steel, titanium) and fiber-reinforced composite posts. Fiber posts are increasingly preferred due to their closer modulus of elasticity to natural dentin, which helps in reducing stress concentration and potential root fracture.

Postandcore restorations are indicated when extensive coronal destruction precludes simply placing a crown directly onto a tooth. Proper technique and case selection are essential to avoid complications such as post fracture, root perforation, or secondary caries. When successfully applied, postandcore can significantly prolong the functional life of custodial teeth and improve restorative outcomes.
