NAPQImetabolii refers to the metabolites of N-acetyl-p-benzoquinone imine (NAPQI). NAPQI is a toxic intermediate produced during the metabolism of acetaminophen (paracetamol) in the liver. Under normal therapeutic doses of acetaminophen, NAPQI is rapidly conjugated with glutathione, a naturally occurring antioxidant, and detoxified. This glutathione conjugate is then further metabolized into various compounds, including mercapturic acids and cysteine conjugates. These detoxified NAPQI metabolites are generally considered less harmful than NAPQI itself and are excreted from the body.
