NAPQImetabolii

NAPQImetabolii refers to the metabolites of N-acetyl-p-benzoquinone imine (NAPQI). NAPQI is a toxic intermediate produced during the metabolism of acetaminophen (paracetamol) in the liver. Under normal therapeutic doses of acetaminophen, NAPQI is rapidly conjugated with glutathione, a naturally occurring antioxidant, and detoxified. This glutathione conjugate is then further metabolized into various compounds, including mercapturic acids and cysteine conjugates. These detoxified NAPQI metabolites are generally considered less harmful than NAPQI itself and are excreted from the body.

However, when acetaminophen is taken in overdose, the liver's glutathione stores become depleted. This leads to an accumulation of NAPQI, which can then bind to cellular proteins, causing oxidative stress and liver cell damage. The NAPQI-protein adducts can undergo further metabolic transformations, leading to a different set of metabolites than those formed from the glutathione conjugate pathway. These altered metabolites are thought to contribute to the hepatotoxicity observed in acetaminophen overdose. Research into NAPQImetabolii focuses on identifying and quantifying these various metabolites to better understand the mechanisms of acetaminophen toxicity and to develop potential biomarkers for early detection and diagnosis of liver injury.
