Iskemiahypoksi is a term used to describe tissue injury that results from a combination of reduced blood flow (ischemia) and decreased oxygen availability (hypoxia). It reflects a common mechanism underlying injury in organs such as the brain, heart, and lungs when perfusion or oxygen delivery is compromised. The concept emphasizes that ischemia and hypoxia can occur together or sequentially, and that their combined effect often worsens tissue damage beyond either factor alone.

Causes and pathophysiology: Ischemia decreases the delivery of oxygen and nutrients to tissues, while hypoxia reduces the amount of oxygen available to cells. When both occur, cellular energy production via aerobic metabolism falls, leading to ATP depletion, anaerobic metabolism with lactate accumulation, and acidosis. Ion pump failure causes cellular swelling, calcium overload, and mitochondrial dysfunction. Reperfusion after a period of ischemia can introduce a surge of reactive oxygen species, inflammation, and additional injury.

Clinical implications: The signs and severity depend on the affected organ and the duration of ischemia-hypoxia. Cerebral involvement can cause abrupt neurological deficits; cardiac involvement may present as chest pain or arrhythmias; other organs can show dysfunction such as reduced organ function or organ failure. Diagnosis relies on clinical presentation supported by imaging, laboratory tests (e.g., lactate, arterial blood gases), and organ-specific assessments.

Management principles: Prompt restoration of blood flow and oxygen delivery is central, alongside stabilization of airway, breathing, and circulation. Treatment targets the underlying cause, minimizes reperfusion injury, and supports organ function. Oxygen therapy is used to correct hypoxia when needed, while careful hemodynamic management helps maintain perfusion without causing harm.

Prognosis and prevention: Outcomes depend on the duration and extent of ischemia-hypoxia and the effectiveness of reperfusion. Preventive measures focus on managing cardiovascular and respiratory risk factors, ensuring adequate perfusion, and rapid response to acute events.
