Ventilaatorireiim, also known as ventilator-induced lung injury (VILI), is a condition that occurs when the use of mechanical ventilation in critically ill patients leads to damage to the lungs. This damage can result from the direct effects of ventilation, such as barotrauma, volutrauma, and atelectrauma, as well as indirect effects like inflammation and oxidative stress. VILI is characterized by the development of acute lung injury (ALI) or acute respiratory distress syndrome (ARDS) in patients who are already receiving mechanical ventilation.

The incidence of VILI varies depending on the population studied, but it is estimated to occur in 10-30% of mechanically ventilated patients. Risk factors for VILI include prolonged ventilation, high tidal volumes, low plateau pressures, and the use of high-frequency oscillatory ventilation. Early recognition and management of VILI are crucial for improving patient outcomes. Treatment strategies focus on minimizing further lung injury, such as reducing tidal volumes, avoiding high plateau pressures, and using protective lung strategies.

Protective lung strategies, such as low tidal volume ventilation and recruitment maneuvers, have been shown to reduce the incidence and severity of VILI. Additionally, the use of lung-protective ventilation strategies has been associated with improved patient outcomes, including reduced mortality and shorter lengths of stay in the intensive care unit. Further research is needed to better understand the pathophysiology of VILI and to develop more effective prevention and treatment strategies.
