Prearthritic is a term used in medicine to describe signs, symptoms, or structural changes in a joint that precede clinically established arthritis. It is mainly employed in rheumatology and orthopedics and often appears in research contexts to denote an early or transitional state that may evolve into osteoarthritis or an inflammatory arthropathy.

The term is applied to individuals who are at risk for arthritis or who show early joint changes that are not yet diagnostic of arthritis by standard criteria. Examples include early cartilage or synovial alterations detected on imaging, intermittent joint pain without radiographic evidence of arthritis, or mechanical joint issues that could progress to degenerative disease. Prearthritic states are heterogeneous and not tied to a single disease.

Clinical features are variable and may include intermittent joint pain, stiffness (often morning or after activity), and occasional swelling. Imaging can reveal non-specific findings such as cartilage thinning, mild osteophyte formation, synovial thickening, or bone marrow lesions, while plain radiographs may be normal.

Diagnosis relies on clinical assessment, imaging, and sometimes biomarkers to differentiate prearthritic conditions from established arthritis. Management focuses on risk reduction and symptom control, including physical therapy, weight management, activity modification, and analgesia. In some cases, close monitoring is used to detect progression toward arthritis.

Prognosis varies; some prearthritic states remain stable, while others progress to osteoarthritis or inflammatory arthritis if risk factors persist or evolve. The term emphasizes the importance of early recognition and preventive care in joint health.
