Postherpetisk refers to conditions that occur after an episode of herpes zoster, commonly resulting in ongoing nerve-related symptoms once the shingles rash has resolved. The most well-known manifestation is postherpetic neuralgia (PHN), a persistent neuropathic pain in the skin area previously affected by the rash.

Causes and risk factors include nerve damage caused by the varicella-zoster virus reactivation during shingles. Factors that increase the likelihood of developing postherpetisk conditions, particularly PHN, include older age (typically over 50), severe initial pain or extensive rash, ophthalmic involvement, diabetes, and immunosuppression. The risk is highest in the weeks to months after the acute infection, but symptoms can persist longer in some individuals.

Symptoms of postherpetisk conditions vary but often center on chronic pain in the affected dermatomal distribution. Patients may experience burning, shooting, or electric shock-like sensations, allodynia (pain from normally nonpainful stimuli), and sensory changes such as numbness or tingling. The onset and duration depend on the severity of the acute episode and the individual’s healing process.

Diagnosis is usually clinical, based on a history of prior shingles and persistent pain beyond the rash resolution. PHN is typically considered if pain lasts more than three months after rash onset. Other causes of facial or body pain may be considered if the presentation is atypical, and a clinician may perform a focused examination to exclude other conditions.

Treatment focuses on pain relief and functional improvement. Common approaches include anticonvulsants (eg, gabapentin, pregabalin), antidepressants (eg, duloxetine, amitriptyline), topical therapies (lidocaine patches, capsaicin), and sometimes short courses of analgesics or opioids for breakthrough pain. Nonpharmacologic options such as physical therapy, cognitive behavioral therapy, and nerve blocks may be used in selected cases. Vaccination against shingles with contemporary vaccines reduces the risk of initial infection and subsequent postherpetic complications.
