Nonurothelial is a term used in pathology and urology to describe tumors arising in the urinary tract that do not originate from urothelial (transitional) epithelium. In the bladder, nonurothelial tumors are a minority compared with urothelial carcinoma and include squamous cell carcinoma, adenocarcinoma, small cell/neuroendocrine carcinoma, and various sarcomas. Similar nonurothelial histologies can occur in other parts of the urinary tract and surrounding structures.

Common nonurothelial bladder cancers include squamous cell carcinoma, which can be linked to chronic irritation or infection (notably Schistosoma infection in endemic regions); adenocarcinoma, which may be urachal or primary; and rare neuroendocrine tumors such as small cell carcinoma. Urachal adenocarcinoma arises from urachal remnants at the bladder dome and may present with mucinous features. Sarcomas, including leiomyosarcoma and rhabdomyosarcoma, are uncommon and tend to be aggressive. The distribution and behavior of these tumors vary by histology, with prognosis closely tied to stage at diagnosis.

Diagnosis relies on histopathology, typically after cystoscopy and biopsy, with immunohistochemistry to determine lineage. For example, squamous differentiation may show p63 positivity; adenocarcinoma often exhibits CK20 and CEA expression patterns; neuroendocrine tumors show markers such as synaptophysin or chromogranin A; sarcomas show mesenchymal markers. Imaging studies assess the extent and stage of disease.

Treatment is histology- and stage-dependent. Localized nonurothelial lesions may be managed with surgical resection, with bladder-sparing approaches considered in select cases. Urachal adenocarcinoma often requires partial cystectomy with en bloc resection of the urachal tract and umbilicus; other nonurothelial tumors may necessitate radical cystectomy or multimodal therapy with chemotherapy and/or radiotherapy. Prognosis varies widely by histology and stage, with more advanced or high-grade tumors associated with poorer outcomes.

In classification schemes, nonurothelial tumors are distinguished from urothelial carcinoma and are described in bladder cancer guidelines and WHO tumor classifications.
