Further progression can lead to the decay reaching the pulp, the innermost part of the tooth containing nerves and blood vessels.

When caries affects the pulp, it can cause significant pain, inflammation (pulpitis), and potentially infection. This advanced stage often requires more complex treatments such as root canal therapy or extraction.
