Hyperinsulinemia is a medical condition characterized by higher-than-normal levels of insulin in the blood. It most commonly arises in the setting of insulin resistance, where body tissues respond less effectively to insulin and the pancreas compensates by producing more insulin to maintain normal blood glucose levels. It is often associated with obesity, metabolic syndrome, and an increased risk of developing type 2 diabetes.

Other causes include insulin-secreting tumors (insulinomas), certain genetic disorders that affect insulin production or signaling, exogenous insulin administration, and certain medications that increase insulin release. In rare cases, familial or idiopathic hyperinsulinemia may occur due to beta-cell hyperfunction or defects in insulin signaling.

Clinical features of hyperinsulinemia are not specific and depend on the underlying cause. When insulin levels are high with low glucose, hypoglycemia can occur, leading to symptoms such as sweating, trembling, anxiety, confusion, or fainting. In many individuals, hyperinsulinemia is identified through metabolic screening rather than through symptoms alone.

Diagnosis typically involves measuring fasting insulin and C-peptide levels, along with blood glucose, to assess insulin secretion and resistance. Calculations such as HOMA-IR can estimate insulin resistance. If an insulinoma is suspected, imaging studies may be used. Distinguishing endogenous hyperinsulinemia from exogenous insulin administration is an important aspect of evaluation.

Management targets the underlying cause and reduction of insulin resistance. Lifestyle interventions including weight loss, physical activity, and dietary modification are foundational. Medications such as metformin or others used for insulin resistance or type 2 diabetes may be employed, and surgical treatment may be necessary for insulinomas. Prognosis depends on the underlying etiology and response to treatment.
