Feltskjema is a term in Norwegian meaning a form composed of individual fields used to collect structured data from respondents or users. It can exist in digital form on websites and apps or as a paper form. The goal is to gather uniform information that can be easily processed, stored and analyzed.

A feltskjema is typically organized with labels for each field, input controls corresponding to the expected data type (text, number, date, email, select menus, checkboxes, radio buttons), and rules such as required fields, length limits, and format validation. Forms may include grouped sections, help text, default values, and error messages to guide the user. Dynamic or conditional fields may appear or disappear based on earlier answers.

Design considerations include clarity, minimalism, logical flow, and accessibility. Clear labels, sufficient contrast, keyboard navigability and screen-reader compatibility are important. Forms should minimize effort, use progressive disclosure if many questions, and validate data both on the client and server sides to ensure quality and security. Localization addresses language, date formats and address conventions.

Data collected via feltskjema is stored in a backend system, often into databases, and may be subject to privacy laws such as GDPR. Responsible handling includes consent where required, purpose limitation, and secure transmission and storage.
