Bonemodifying is a term used to describe processes and interventions that alter the properties of bone tissue. This can include natural biological remodeling, where bone tissue is formed and resorbed, as well as clinical or experimental methods aimed at changing bone mass, density, geometry, or mechanical strength. The concept covers both physiological adaptation to growth and loading, and medical strategies to treat or prevent bone disorders.

Natural bone modification occurs through the coordinated activity of osteoblasts, osteoclasts, and osteocytes in a remodeling cycle. Hormones (parathyroid hormone, calcitonin, estrogen, thyroid hormone), vitamins (D and A), and mechanical factors influence formation and resorption. Wolff's law describes how bones adapt structurally to habitual loads, while aging and disease can shift the balance toward weaker bone or abnormal mineralization.

In medicine, bonemodifying often refers to agents or procedures that influence bone turnover. Pharmacological bone-modifying agents include bisphosphonates and denosumab (antiresorptives) and anabolic treatments such as teriparatide and romosozumab. Surgical and therapeutic approaches such as bone grafting, distraction osteogenesis, and orthopedic implants also modify bone. In research, bonemodifying strategies include tissue-engineering, gene therapy, and biomaterial scaffolds to promote bone formation or healing.

Pathologies associated with altered bone modification include osteoporosis, osteomalacia, osteogenesis imperfecta, and osteopetrosis. Assessment of bonemodifying effects uses imaging techniques like dual-energy X-ray absorptiometry (DXA), X-rays, computed tomography, and bone turnover markers in blood or urine. Safety considerations include risks associated with pharmacologic agents and procedures, such as osteonecrosis of the jaw or atypical femoral fractures with prolonged antiresorptive use, requiring medical supervision.
