Wastewater-based analysis refers to methods that examine municipal wastewater to infer information about the health, behavior, or exposures of the population contributing to the sewer system. The approach encompasses chemical markers such as pharmaceuticals and illicit drugs, as well as biological markers including pathogens and biomarkers. By aggregating data at the community level, wastewater-based analysis provides anonymized, population-scale insight without surveying individuals.

Origins of wastewater-based approaches date to environmental and pharmaceutical monitoring in the late 20th century, but they gained prominence for public health in the 21st century, particularly during the COVID-19 pandemic when SARS-CoV-2 RNA was detected in wastewater as an early indicator of community spread.

Typical methods combine wastewater sampling (composite samples over time versus single grab samples) with laboratory concentration and detection. Chemical analyses commonly use LC-MS/MS or GC-MS; pathogen surveillance often uses qPCR or sequencing. Data are normalized using wastewater flow and population size to estimate per-capita loads and trends.

Applications include monitoring drug use patterns, tracking infectious diseases, assessing exposure to contaminants, and supporting outbreak detection and environmental risk assessments.

Advantages include noninvasive, anonymized, and cost-effective population-level surveillance with potential for early warnings. Limitations involve representativeness, variability in diurnal flow, degradation of compounds, sampling biases, and uncertainties in back-calculation to prevalence.

Challenges include standardization of methods, data interpretation, and ethical considerations related to privacy. Ongoing research aims to harmonize protocols, improve models, and integrate wastewater data with clinical and environmental information for public health decision making.
