Underactivation

Underactivation refers to a state in which activity in a given system is lower than the level required for normal function or expected task performance. The term is used across fields to describe insufficient activation, whether of neural circuits, muscles, hormonal systems, or cognitive processes.

In neuromuscular contexts, underactivation describes when motor units or muscles fail to reach adequate activation during voluntary movement. This can contribute to weakness, slowed movements, or impaired coordination and can arise from neural inhibition, fatigue, disuse, injury, or aging. In neuroimaging, underactivation describes regions that show less activity during a task than would be predicted by healthy controls; interpretations vary and may indicate reduced processing efficiency, structural disruption, or compensatory recruitment of other regions.

In psychiatry and psychology, patterns of underactivation have been reported in reward or executive networks in certain disorders, while in aging and cognitive tasks, underactivation can reflect diminished resources or strategic differences rather than deficit alone.

In endocrinology or immunology, underactivation can refer to reduced hormonal signaling or immune responsiveness, which may occur due to chronic stress, disease, medications, or circadian disruption.

Causes are multifactorial and can include disuse, injury, aging, neuropathology, metabolic factors, medications, and stress. Consequences include impaired task performance, slower recovery after injury, or misinterpretation of imaging results.

Assessment relies on objective measures such as electromyography for muscle activation, functional imaging (fMRI, PET), electrophysiological testing, and task performance metrics. Interventions target the underlying cause and may include rehabilitation and strength training, neuromodulation, cognitive and behavioral strategies, pharmacologic adjustments, and optimization of sleep and stress management.
