It encompasses the complete life cycle of a patient encounter from scheduling and registration through charge capture, claims submission, remittance processing, denial management, patient collections, and reporting. RCM aims to maximize financial performance while maintaining compliance with billing and healthcare regulations and ensuring timely access to care for patients.

Process flow typically begins with patient scheduling and eligibility verification, followed by registration, accurate charge capture and coding (using ICD-10-CM/PCS, CPT codes), and the creation of a claim. Claims are submitted to payers, adjudicated, and remittance advice is reconciled. Denied or partially paid claims trigger denials management and appeal workflows. Payments are posted, patient balances are billed, and collections activities proceed as needed. Throughout, data exchanges between practice management systems, electronic health records, and payer portals support tracking and reporting.

Key components include code sets and documentation, the charge description master, payer contracts, claims management, denial management, patient billing and self-pay collections, and analytics. Common performance metrics include net collection rate, days in accounts receivable, denial rate, clean claim rate, medication and services mix, and cash flow projections. RCM may be supported by software platforms, outsourcing arrangements, or integrated suites that connect to hospitals or clinics' EHRs and billing systems.

RCM is central to the financial health of healthcare providers and affects patient access, compliance, and risk management. It must adapt to payer requirements, evolving regulations, and changes in clinical documentation. Challenges include high denial rates, complex payer rules, varying contract terms, and the need for accurate coding and timely collections.
