Päävammat, which translates from Finnish as "head injuries", encompass a spectrum of traumatic brain incidents ranging from mild concussions to severe acute brain trauma. They can be classified into closed head injuries, when the skull remains intact, and penetrating injuries, where an external object breaches the skull. The majority of cases arise from falls, motor‑vehicle collisions, sports accidents, and violence such as assaults or gunshot wounds.

Symptoms vary with severity. Mild forms often present with transient loss of consciousness, confusion, headache, nausea, and brief memory gaps. Moderate to severe injuries may involve prolonged unconsciousness, seizures, neurological deficits, impaired speech, and loss of bodily functions. Physical examination and neuroimaging—computed tomography (CT) for acute bleeding and magnetic resonance imaging (MRI) for diffuse injuries—are standard diagnostic tools. Neurological assessment scales such as the Glasgow Coma Scale assist in estimating injury severity.

Immediate management focuses on ensuring airway patency, breathing adequacy, and circulatory stability while preventing secondary brain injury from hypoxia, hypotension, or increased intracranial pressure. Specific treatments include anticonvulsants for seizures, osmotic diuretics to lower intracranial pressure, and surgical intervention for mass effect or hematomas when indicated. Rehabilitation may involve physical therapy, occupational therapy, speech therapy, and psychological support.

Prevention strategies emphasize the use of helmets in cycling, motorcycling, and contact sports, seat‑belt and child‑seat installation in vehicles, and workplace safety regulations. Educational campaigns targeting risk factors such as alcohol misuse and non‑use of protective gear reduce the incidence of severe head injuries. Epidemiological data from national health registries indicate that head injuries constitute a significant proportion of traumatic morbidity, with particular burden in the young adult and adolescent populations.

The literature encourages the adoption of evidence‑based guidelines for assessment and treatment, and continual research into mitigating secondary injury pathways, enhancing neuroregeneration, and improving long‑term functional outcomes.
