Neuropathologic

Neuropathologic is an adjective relating to neuropathology, the branch of pathology that studies diseases of the nervous system tissue, including the brain, spinal cord, peripheral nerves, and the eye. It encompasses the macroscopic and microscopic evaluation of tissue, focusing on structural and cellular alterations that reflect disease processes.

In practice, the neuropathologic assessment often relies on tissue obtained at autopsy, though biopsy specimens may be used in certain clinical contexts. The process includes gross inspection, sampling, histologic processing, and staining to reveal neuronal loss, gliosis, plaques, tangles, inclusion bodies, demyelination, vascular pathology, and neoplastic changes. The goal is to establish a disease signature that supports diagnosis, classification, and research interpretation.

Techniques commonly employed include histology with routine stains, special stains for myelin and calcifications, and immunohistochemical assays to detect disease-related proteins such as beta-amyloid, phosphorylated tau, alpha-synuclein, TDP-43, and huntingtin. Molecular pathology methods, in situ hybridization, and, less often, electron microscopy or genetic testing, complement morphological assessment and help refine nosology and etiologic hypotheses.

Neuropathology informs the diagnosis of neurodegenerative disorders (e.g., Alzheimer's disease, Parkinson's disease, frontotemporal dementia), prion diseases, cerebral vascular disorders, infectious diseases, demyelinating conditions, and brain tumors. It contributes to staging and research correlates, supporting the development of classification criteria and therapeutic understanding.

Limitations include postmortem interval effects, tissue preservation, sampling bias, and interpretive variability. Ongoing standardization, brain banks, and consensus criteria aim to enhance reliability and cross-study comparability in neuropathology.
