Demyelinating refers to the process or condition of demyelination, the loss or destruction of the myelin sheath that surrounds nerve fibers in the central nervous system (CNS) or peripheral nervous system (PNS). Myelin accelerates the transmission of electrical signals along axons; when it is damaged, conduction slows or fails, leading to a range of neurological symptoms.

Demyelination can result from inflammatory, autoimmune, metabolic, infectious, or traumatic processes. In the CNS, common demyelinating conditions include multiple sclerosis, acute disseminated encephalomyelitis, and neuromyelitis optica spectrum disorder. In the PNS, inflammatory demyelinating diseases include Guillain-Barré syndrome and chronic inflammatory demyelinating polyneuropathy. Central pontine and other metabolic demyelinations can occur in contexts such as severe electrolyte disturbances or osmotic shifts. Remyelination by glial cells is possible in some cases, which can restore function partially, though recurrent injury often leads to permanent axonal loss.

Clinical features depend on the areas affected and can include sensory disturbances, weakness, impaired coordination, visual problems, or autonomic dysfunction. Diagnosis typically relies on clinical presentation supported by imaging and laboratory tests: MRI can reveal demyelinating lesions in the CNS; nerve conduction studies assess PNS involvement; cerebrospinal fluid analysis may show inflammatory markers or oligoclonal bands. In uncertain cases, brain or nerve biopsy may be performed.

Treatment focuses on the underlying cause and symptom management. Acute demyelinating events often respond to corticosteroids or plasmapheresis. Long-term management may include disease-modifying therapies for CNS demyelinating diseases, immunoglobulin therapies for certain neuropathies, and rehabilitation to address functional deficits. Prognosis varies widely depending on the condition and extent of axonal damage.
