Methylphenidate-based products refer to medicines containing methylphenidate or its active enantiomer dexmethylphenidate, or a combination of both, and are used primarily to treat attention deficit hyperactivity disorder (ADHD) and narcolepsy. The drug acts by blocking the reuptake of dopamine and norepinephrine, increasing their concentration in the synaptic cleft and improving attention, impulse control, and, in some cases, wakefulness.

Methylphenidate is typically started at a low dose and titrated to achieve therapeutic effect. Formulations differ in duration: immediate-release (short-acting, typically several hours) and extended-release (long-acting, often 8–12 hours) preparations. Osmotic release systems, extended-release capsules, tablets, and transdermal patches are among the delivery methods. Examples of methylphenidate-based products include methylphenidate hydrochloride formulations such as Ritalin, Concerta, Metadate, and Methylin, as well as the transdermal Daytrana. Dexmethylphenidate-based products, such as Focalin and Focalin XR, use the more potent enantiomer.

Racemate dl-methylphenidate is the traditional form, whereas dexmethylphenidate is the isolated more active enantiomer used in separate products.

Safety and monitoring are important: common adverse effects include insomnia, reduced appetite, abdominal pain, headache, tachycardia, and elevated blood pressure. There is potential for misuse and dependence, and stimulant use is generally contraindicated in certain cardiovascular conditions, a history of substance use disorders, and, with caution, in some psychiatric conditions. Pregnancy or breastfeeding requires careful risk-benefit evaluation. Monitoring should cover growth in children, cardiovascular status, sleep, and mood.

Regulatory status varies by country, but methylphenidate-based medicines are controlled substances in many jurisdictions (for example, Schedule II in the United States). Availability as generics and brand-name products varies by market.
