Immediate-release (IR) refers to pharmaceutical dosage forms designed to release the active drug promptly after administration, enabling rapid absorption and onset of action. IR formulations include tablets, capsules, liquids, powders, and injectables designed to dissolve quickly, delivering therapeutic levels soon after dosing. The term contrasts with delayed-release, extended-release, and sustained-release formulations, which are engineered to release drug over a longer period.

In pharmacokinetics, IR products typically produce a fast onset of action, with maximum plasma concentration reached within about 0.5 to 2 hours, depending on the drug and route of administration. Because the drug is released quickly, concentrations rise and fall more rapidly than with controlled- or extended-release forms, often necessitating more frequent dosing and greater fluctuations in drug levels. This can be advantageous for acute symptoms, urgent relief, or diagnostic testing, but may increase the risk of peak-related adverse effects.

Common examples include immediate-release analgesics (such as many acetaminophen tablets), certain oral antibiotics, and various non-prescription remedies. IR is frequently chosen when rapid symptom relief is desired, when drug exposure must be tightly controlled, or when formulations with coatings or matrices are unsuitable for the patient.

Regulatory labeling distinguishes IR from other release mechanisms, and product development teams select IR versus ER based on therapeutic goals, safety, stability, and patient adherence. Both forms may be available for the same active pharmaceutical ingredient, offering clinicians options for dosing schedules and clinical scenarios.
