Juurisyyanalyysi, or root cause analysis (RCA), is a systematic problem-solving method used to identify the underlying causes of an incident or problem. The primary goal of RCA is not just to address immediate symptoms but to prevent recurrence by finding and eliminating the fundamental reasons. This process involves a thorough investigation, often beginning with a clear definition of the problem or incident that occurred.

Several methodologies and tools can be employed in a root cause analysis. A common technique is the "5 Whys," where one repeatedly asks "why" until the fundamental cause is uncovered. Other methods include fishbone diagrams (Ishikawa diagrams), fault tree analysis, and Pareto charts. These tools help to organize data, identify potential causes, and prioritize the most significant ones.

The RCA process typically involves several stages. First, the problem is defined and investigated to gather relevant data. Second, causal factors are identified, which are the events or conditions that directly contributed to the problem. Third, causal chains are developed to link these factors back to their origins. Fourth, the root cause(s) are determined, representing the fundamental reason(s) that, if removed, would prevent the problem from happening again. Finally, recommendations for corrective and preventive actions are developed and implemented.

Root cause analysis is widely used across various industries, including manufacturing, healthcare, aviation, and IT. Its application helps organizations improve safety, enhance efficiency, reduce costs, and learn from mistakes.
