Intracerebral is an anatomical term that describes something situated within the brain parenchyma, the functional tissue of the brain. It is used to distinguish processes occurring inside the brain itself from those in surrounding spaces such as the subarachnoid space, ventricles, or skull.

The phrase is most commonly encountered in reference to intracerebral hemorrhage, which denotes bleeding within brain tissue. It is also applied to intracerebral lesions and conditions that arise within the brain parenchyma, including tumors, abscesses, and areas of infarction with hemorrhagic transformation. The term helps specify the location of pathology as opposed to intracranial processes that may involve the meninges, ventricles, or subarachnoid spaces.

Etiology and risk factors for intracerebral processes are diverse. Intracerebral hemorrhage, a major context for the term, is often caused by hypertension, cerebral amyloid angiopathy, trauma, ruptured vascular malformations, or coagulation disorders. Other intracerebral conditions may result from infection, neoplastic growth, autoimmune disease, or stroke with secondary bleeding. Risk factors commonly overlap, including age, hypertension, alcohol use, and anticoagulant or antiplatelet therapy.

Clinical presentation varies with location and cause but typically includes sudden neurological deficits such as weakness or numbness, speech or vision changes, headache, vomiting, or altered consciousness. Diagnostic evaluation relies on imaging, with non-contrast CT scans used acutely to detect hemorrhage and MRI providing detailed characterization of tissue, edema, and underlying causes.

Management is condition-specific and ranges from blood pressure control and supportive care to neurosurgical intervention for certain large or obstructing hematomas, treatment of infections or tumors, and targeted therapy for the underlying etiology.
