Hændelsesanalyse

Hændelsesanalyse, often translated as incident analysis or event analysis, is a systematic process used to investigate adverse events, accidents, or near misses. The primary goal is to understand the root causes and contributing factors that led to the incident, rather than simply identifying blame. This understanding is then used to implement corrective and preventive actions to reduce the likelihood of similar events occurring in the future.

The process typically involves several stages. First, an incident is identified and reported. This is followed by data collection, which may include interviews with involved parties, examination of physical evidence, review of documentation, and observation of the environment. Once the data is gathered, analysis is performed to determine the sequence of events and identify the underlying causes. Common analytical tools include root cause analysis (RCA), fishbone diagrams (Ishikawa diagrams), and the "five whys" technique.

The findings from the analysis are then documented, and recommendations for improvement are developed. These recommendations might involve changes to procedures, training, equipment, or organizational policies. Finally, the implemented changes are monitored to ensure their effectiveness. Hændelsesanalyse is widely applied in various sectors, including healthcare, aviation, manufacturing, and IT, to enhance safety, improve quality, and learn from mistakes.
