Behandlungsplan

Behandlungsplan is a structured, written plan guiding a patient’s medical or therapeutic care. It is typically developed by the treating team together with the patient and/or guardians and documents problems, goals, interventions, and a timetable.

Its aim is to coordinate care across settings, ensure transparency and continuity, and support resource planning. Behandlungspläne are used in hospitals, outpatient clinics, rehabilitation, and long-term care across disciplines such as medicine, dentistry, psychology, and nursing.

A plan usually includes a problem list or diagnosis, treatment goals (often SMART), planned interventions (tests, medications, procedures, therapies), a timeline with milestones, assigned responsibilities, and a plan for monitoring, evaluation, and possible revision.

Development begins with assessment and patient involvement, with consent and preferences recorded. It is typically created by a multidisciplinary team, documented in the health record, and reviewed regularly or when the condition changes.

Behandlungspläne support evidence-based, patient-centered care, improve continuity, and enable accountability. They rely on confidentiality and data protection and may be aligned with national guidelines or care pathways.

In chronic disease management, a Behandlungsplan may specify targets (e.g., blood pressure, glycemic control), prescribed therapies, lifestyle recommendations, follow-up dates, and criteria for adapting or terminating the plan.
