Valgus stress test is a medical diagnostic procedure used to assess the integrity of the medial collateral ligament (MCL) and the lateral collateral ligament (LCL) of the knee joint. The test is performed arthroscopically or using fluoroscopic imaging to apply a valgus stress to the knee joint.

The procedure involves the patient lying on a knee table with the affected leg hanging off the end of the table. The examiner applies a valgus force to the knee, which stretches the MCL and may also affect the LCL. The force is typically applied at different points in the knee's range of motion to assess the ligament's tightness and stability.

A positive result is indicated when the valgus force causes excessive movement or laxity of the knee joint, suggesting an injury to the MCL or LCL. The test is specific for detecting medial and lateral collateral ligament injuries but not as sensitive for detecting injuries to the anterior cruciate ligament (ACL).

The valgus stress test is commonly used in sports medicine and orthopedic clinics to diagnose and monitor the treatment of knee injuries, particularly in athletes who participate in sports that involve twisting, pivoting, and sudden changes in direction, such as soccer, basketball, and football. Interpretation of the results must be done in conjunction with other diagnostic tests, such as arthroscopy, MRI, and clinical examination, to confirm the diagnosis.
