Undermineralized is an adjective used to describe tissue that contains less mineral content than normal for its developmental stage or anatomical site. It is a term encountered in dental and skeletal contexts, where mineral deposition is essential for hardness and structural integrity.

In dental science, undermineralization most often refers to enamel or dentin that has not achieved typical mineral deposition during formation, resulting in softer areas that are more susceptible to wear, hypersensitivity, and caries. This can occur with disrupted amelogenesis or dentinogenesis and is sometimes discussed in relation to conditions such as hypomineralization or amelogenesis imperfecta variants.

In bone and cartilage, undermineralization describes reduced mineral deposition in the extracellular matrix, producing bones that are lighter in density and more prone to fracture. It can arise from nutritional deficiencies, metabolic disturbances, endocrine disorders, chronic illness, or genetic defects affecting mineralization processes.

Causes of undermineralization are varied and may include insufficient intake or absorption of calcium, phosphate, or vitamin D; disorders of mineral metabolism (e.g., hypophosphatemia, renal phosphate handling issues); endocrine dysfunction; chronic systemic diseases; and certain genetic conditions that affect enamel, dentin, or bone formation. Medications and lifestyle factors can also influence mineralization.

Diagnosis typically combines clinical examination with radiographic or histological assessments to detect reduced mineral density or mineral-to-organic matrix ratios. In systemic cases, bone mineral density testing may be informative.

Treatment targets the underlying cause and may involve nutritional and metabolic correction, supplementation of deficient minerals, and targeted dental or orthopedic interventions to restore function and protect affected tissues. Prognosis varies with severity and etiology.
