Thyroglossal is an anatomical and pathological descriptor relating to the thyroid gland and its embryologic tract, particularly the thyroglossal duct and any remnants along its course.

Embryology and anatomy: The thyroid originates near the base of the tongue at the foramen cecum and descends to the neck. The thyroglossal duct is the transient conduit that connects the developing thyroid to the tongue; it normally involutes by late gestation. Remnants of the duct can persist anywhere along the descent path, sometimes giving rise to cysts or ectopic thyroid tissue.

Clinical features: Thyroglossal duct remnants most commonly present as midline neck lesions. A thyroglossal duct cyst typically appears as a painless swelling that moves upward with swallowing or tongue protrusion. Infections, inflammation, or fistulization can occur, particularly in childhood or adolescence.

Diagnosis: Initial evaluation often uses ultrasound, especially in children, to assess the cyst and to identify any orthotopic thyroid tissue. In adults, cross-sectional imaging (CT or MRI) may delineate anatomy and rule out alternative neck masses. Nuclear medicine scans or thyroid function tests can verify the presence of functioning thyroid tissue elsewhere in the neck.

Treatment and prognosis: Symptomatic thyroglossal duct cysts are typically treated with surgical excision via the Sistrunk procedure, which removes the cyst, the entire thyroglossal tract, and a central portion of the hyoid bone to reduce recurrence. Prognosis after complete excision is generally good. Rarely, carcinoma (most often papillary thyroid carcinoma) can arise in a thyroglossal duct cyst, potentially altering management.

Etymology: The term combines thyro- (thyroid) with glossa (tongue), reflecting the duct’s origin and path from the tongue toward the thyroid.
