Spierknop is a term used in Dutch to describe a localized, palpable lump or knot within a skeletal muscle, commonly associated with increased tension. In many cases it corresponds to a myofascial trigger point, a hypersensitive spot within a taut band of muscle fibers.

Symptoms include a tender nodule that can be felt under the skin, muscle stiffness, and sometimes referred pain to other areas. When pressed, the knot may reproduce or intensify pain, and the surrounding muscle may react with guarding or twitching.

Causes include repetitive strain, overuse, prolonged poor posture, muscle fatigue, stress, injury, or inactivity. Myofascial trigger points may develop when muscle fibers fail to relax after overload, leading to a persistent state of hyperirritability within the muscle.

Diagnosis is clinical, based on palpation and symptom patterns. Imaging is rarely needed unless there are red flags or atypical features to exclude other conditions such as lipomas, cysts, or nerve lesions.

Treatment aims to reduce pain and restore normal muscle function. Approaches include physical therapy with stretching and strengthening, manual therapy, myofascial release, dry needling or injections for persistent cases, and self-management with heat, cold, gentle massage, and gradual return to activity.

Prognosis varies; many cases improve with treatment, though episodes can recur. Prevention focuses on regular movement, posture correction, ergonomics, balanced training, and adequate recovery.
