Smitleiðir are the pathways by which an infectious agent is transmitted from an infected source to a susceptible host. In public health, understanding smitleiðir is essential for preventing spread and for guiding outbreak response, since interventions are often most effective when they target the specific routes involved.

Common smitleiðir include direct contact (person-to-person touch), indirect contact (via contaminated objects or surfaces, or fomites), droplet transmission (large respiratory droplets from coughing or sneezing within about a meter), airborne transmission (smaller particles that can travel longer distances and remain suspended), vector-borne transmission (via insects or other animals), foodborne and waterborne transmission (ingestion of contaminated food or water), vertical transmission (from mother to fetus or newborn), and zoonotic transmission (from animals to humans).

Transmission is influenced by pathogen factors (infectious dose, environmental stability), host factors (immunity, behavior), and environmental conditions (ventilation, crowding, sanitation). Public health measures aim to interrupt specific routes, for example through hand hygiene and personal protective equipment to reduce direct or indirect contact, cleaning and disinfection of surfaces, respiratory precautions and improved ventilation for droplet and airborne routes, vaccination, safe food and water practices, vector control, and appropriate isolation or quarantine when necessary.

Surveillance, contact tracing, outbreak investigations, and epidemiological modeling help identify dominant smitleiðir in a given setting and guide targeted interventions to reduce transmission and protect populations.
