Noninflammatory is an adjective used in medicine and biology to describe processes, conditions, or tissue responses that do not involve inflammation. Inflammation is a complex protective response that typically features redness, heat, swelling, pain, and sometimes fever, driven by immune cells and signaling molecules. Noninflammatory conditions, by contrast, show little to no leukocyte infiltration and often normal systemic inflammatory markers in blood tests.

The distinction is not always absolute. Some diseases can be largely noninflammatory in their dominant pathology but may develop inflammatory features at certain stages or in subsets of patients. Similarly, an event may be noninflammatory at onset but evolve to involve inflammation later.

Common examples of noninflammatory conditions include degenerative joint disease (osteoarthritis), certain tendinopathies, and other degenerative or mechanical conditions that arise from wear and tear rather than immune-mediated processes. In these cases symptoms may reflect structural damage, nerve irritation, or altered biomechanics rather than classic inflammatory mediators.

Clinical evaluation typically relies on history, physical examination, and laboratory tests for inflammatory activity (such as C-reactive protein and ESR) that are often normal in true noninflammatory conditions. Imaging and, when needed, tissue analysis can help distinguish noninflammatory degeneration from inflammatory or autoimmune processes.

Treatment focuses on symptom relief and underlying mechanical or degenerative issues, rather than anti-inflammatory strategies, though drugs with anti-inflammatory effects may be used if inflammation is present or uncertain.
