Malignoituminen is the term used in Finnish medical language for the process by which a benign or pre-cancerous lesion transforms into a malignant neoplasm. It describes the acquisition of cancerous characteristics such as unregulated growth, invasion into surrounding tissues, and the potential to spread to distant sites. In English-language literature the concept is usually called malignant transformation or malignant degeneration.

The process involves genetic and epigenetic changes that accumulate over time. Mutations in oncogenes and tumor suppressor genes, along with chromosomal instability and altered DNA repair, drive clonal evolution of cells with progressively malignant traits. Changes that promote evasion of apoptosis, sustained angiogenesis, and escape from immune surveillance contribute to the progression from a non-cancerous state to invasive cancer.

Common pathways of malignoituminen are described in various organ systems. For example, colorectal cancer often evolves from an adenomatous polyp to carcinoma, while actinic keratoses can progress to squamous cell carcinoma of the skin. Other well-known sequences include Barrett’s esophagus to esophageal adenocarcinoma and precancerous lesions in the liver or cervix progressing to hepatocellular carcinoma or invasive cervical cancer, respectively. Not all benign lesions will become malignant; the probability depends on factors such as tissue type, genetic susceptibility, and exposure to risk factors.

Risk factors include chronic inflammation, exposure to carcinogens (like tobacco or ultraviolet light), viral infections (such as HPV, HBV, HCV), and certain inherited cancer syndromes. Diagnosis relies on morphological criteria of invasion and, increasingly, molecular markers that indicate malignant potential. Prevention and management focus on surveillance, timely removal of precancerous lesions, and, for established cancers, standard oncologic therapies.
