Common causes include high altitude, chronic or acute lung disease, respiratory failure or ventilation-perfusion mismatch, carbon monoxide or methemoglobin poisoning, heart failure, shock, severe anemia, and certain toxins such as cyanide.

Clinical features vary but often include shortness of breath, rapid breathing, rapid heart rate, anxiety, and confusion. In severe cases, cyanosis, loss of consciousness, or seizures may occur.

Diagnosis relies on clinical assessment and laboratory tests such as pulse oximetry (SpO2) and arterial blood gas analysis (PaO2, SaO2), along with complete blood count and tests for hemoglobin function. Lactate levels may rise with tissue hypoxia.

Treatment aims to restore oxygen delivery and treat the underlying cause. It may involve supplemental oxygen, mechanical ventilation, blood transfusion for anemia, antidotes or supportive care for poisoning, and management of cardiovascular instability. Prevention focuses on managing chronic respiratory disease, avoiding high-altitude exposure without acclimatization, and treating conditions that impair oxygen transport.

Prognosis depends on the duration and severity of hypoxia and the underlying condition; with prompt treatment, many reversible cases improve, while prolonged hypoxia can cause lasting organ damage.
