A hydrocele is a collection of fluid within the sac that surrounds a testicle. In infants, hydroceles are quite common and usually resolve on their own within the first year of life. They can be congenital, meaning present at birth, or acquired later in life. Acquired hydroceles can be caused by various factors, including inflammation, infection, or trauma to the scrotum.

The primary symptom of a hydrocele is a painless swelling in one or both sides of the scrotum. The swelling may be more noticeable in the morning and decrease throughout the day. While usually not painful, a large hydrocele can cause a feeling of heaviness or discomfort. Diagnosis is typically made through a physical examination. Sometimes, an ultrasound may be used to confirm the presence of fluid and rule out other conditions.

In most infant cases, no treatment is necessary as the hydrocele will disappear naturally. For older children and adults, if the hydrocele persists, is large, causes discomfort, or is associated with other symptoms, surgical intervention may be recommended. Surgery, known as a hydrocelectomy, involves removing the sac containing the fluid. The prognosis for hydroceles is generally very good, especially in infants.
